The thing is, honestly, this could happen to any given plane on any given day. People don't realize how little has to happen for cracks and imperfections in the walls of the plane to propagate and become holes. That being said, planes and all other objects designed for human contact or use are designed with built in factors of safety to ensure that the maximum stresses are never experienced. Considering the number of deaths and injuries associated with flying every year, it's extremely safe. My point is that Southwest is not necessarily at fault, as the material used to build the fuselage could have contained undetectable cracks or imperfections of sizes large enough that they would not have been planned for.
